Abstract

The synthesis of new pharmacological molecules and their introduction into medical practice constitute a stringent, multistage, and complex fundamental system governed by international standards. Preclinical (in vitro, in vivo) and clinical trials (Phases I-IV), which determine the safety profile and therapeutic window of drugs, serve as the primary shield protecting patient lives. This study aims to comprehensively analyze the translational pathway of pharmaceutical agents from the laboratory bench to clinical application, addressing the ensuing pharmacokinetic and pharmacodynamic barriers alongside the molecular mechanisms required to overcome them. Based on prospective and retrospective analyses, the 5-year testing trajectory (2020-2025) of 120 novel pharmacological candidate molecules was investigated. During the preclinical stage, conducted under Good Laboratory Practice (GLP) standards, 70.8% of the molecules were rejected due to profound toxicity (unsatisfactory LD50 parameters) and poor bioavailability. In the clinical trials, executed in accordance with Good Clinical Practice (GCP) requirements, only 7.5% of the initial candidates successfully navigated all phases (I, II, III) to attain regulatory approval. Furthermore, 45% of the attrition observed in Phase II clinical trials was attributed to unanticipated hepatotoxic reactions within the human biological system and sub-optimal affinity for target receptors. The application of pharmacogenetic approaches and the consideration of individual metabolic phenotypes during drug testing augmented clinical trial efficacy by 28%, a finding confirmed via mathematical regression models. The results of this investigation establish a highly informative platform for implementing systemic modifications to the pharmacological screening algorithm and accurately assessing the risk-benefit ratio, thereby optimizing the duration and economic expenditures of clinical trials.
