Abstract

Metabolic dysfunction–associated steatohepatitis (MASH) is the progressive inflammatory form within the spectrum of metabolic dysfunction–associated steatotic liver disease (MASLD), strongly linked to fibrosis progression, cirrhosis and hepatocellular carcinoma. Terminology and diagnostic strategies have recently evolved, shifting the focus toward early identification of “at-risk” patients (≥F2 fibrosis) through non-invasive tests (NITs), appropriate referral pathways, and longitudinal monitoring. This review summarizes evidence on MASH pathogenesis and clinical phenotypes, blood-based scores (FIB-4, NFS), elastography (VCTE), MRI-based tools (MRI-PDFF, MRE), serum biomarkers (ELF, PRO-C3), and combined diagnostic algorithms. A pragmatic stepwise optimization model is proposed: risk stratification → secondary assessment → specialist referral.
